Help! Fuel leak with NOS safety switch

Ok, I'm in the middle of installing my NOS dry kit. I drilled and tapped the banjo bolt just like the instructions say for installing the NOS fuel pressure switch but now I get a small gas leak from that area when the car is running. I double-checked my helms and I have everything in order, including the two washers. I also used teflon paste on the connections.

I can't tell which connection the leak is from since it just drips everywhere. Anyone else had this problem? I know the banjo bolt was torqued to 25 lb-ft. How tight does the 1/8" NPT adapter that fits into the tapped banjo bolt hole need to be? Damn I hate this part of the install.

The washers are crush washers, so yes, you need a new one.
