tranny issuse

So i got my prelude running and it was shifting hard so I added tranny fluid and it fixed it and then i got about 30 miles on it and then it started to slip so I added more. Fixed it till I got home, it still shifts fine but it jerks really hard into reverse but not drive it does fine in drive. the tranny was very hot. do you think that if I do a flush and fill that I could save it? or do you think that it is screwed? Its an auto with 108k and with a JDM h23.

Also its a 96, and I have a check engine, where is the port to read codes?

The OBD2 connector is under/behind the cupholder pocket thingy in the center console. At least on my 96, which I thought was strange, but is actually more convenient than under the dash.
