trans slips out of 5th
some noise in 5th, then in lower gears also.
Did 200 mile trip yesterday was fine till nearly arriving at home.
Seems tight and not misaligned with engine block.

I got no results with search attempt.

Only way to know for sure is to pull it apart and find out, that's how manual transmissions work but if you want a guess it's more than likely the fork as it may be damaged so that the detent ball no longer keeps it locked into gear. You aren't giving enough info but if there is noise from the trans in all gears then you could have some bad bearings, wont know which until you pull it apart.
