problem with my brakes

Ok heres my problem sometimes when im driving the brake pedal will feel soft and mushy and will go all the way to the floor. But this only happens sometimes, and i can also hear some kind of noise that sounds like grinding. what do you ppl think might be the problem? Im gonna bleed the brakes tomorrow a see if that helps. its a 93 vtec if that matters.

ok i changed the master cylinder and the brake pedal has pressure now..but when i push down on the pedal i hear a noise(its not grinding or anything its more of a click sound) any ideas what this is??? I can also feel it when i push the pedal i will apply pressure and i can feel it right as it makes the noise.
