1 - never done it (havent had coolant in there long enough, blew motor once, got new radiator once)
2 - every 3-5k miles
3 - Only did it with the clutch (havent had car long enough, same reason as #1)
4 - 10k+ depending on plugs, iridiums have been in there 25k and still work fine, although gas mileage is only like 25 mpg
5 - never ever done it
1 = Never, someone hit front end and weasled a new rad out of insurance
2 = 3-5k or whenever i can find time
3 = i dunno when the last time was, but i will in about 2-3 weeks
4 = when the engine starts running ****ty
5 = changed with different intake
1 Change Radiator Fluid/Coolant - When necessary.
2 Change Engine Oil - Every 4k-5k miles.
3 Change Transmission Fluid - When mechanic says needed.
4 Change Spark Plugs - Every 10,000 miles.
5 Clean/Change your air filter - Clean every 5k, change with the color fades dramatically.
I like to keep my car running happy.

3 - About 25k ago, should do again soon.
4 - Changed about 20k ago with the wires.

I'd highly recommend changing out your muffler bearings from time to time. i noticed roughly 15 hp to the wheels by removing my muffler bearings and simply relubing them.
Another good power adder would be to properly balance you're twin wampalater shafts. I simply took them out, shaved 1/32nd of an inch off, and put them back in backwards. I'd say i cleared well into 9.75hp to the wheels. Atleast!
i've changed coolant roughly a dozen times. Once with the thermistat. And it use to eat coolant when i had a CEL (i dont know how it went away). So i would flush it out and replensh it in hopes of it one day not eating it. And it worked!
Oil is as often as i can find time for it.
Tranny fluid was only when i rebuilt the motor.
i changed the atts juice just the other week. to that headache.
coolant, plugs, and tranny fluid - probably once every two years. Maybe more for the tranny.
Oil - every 5000 or so. Depending on driving habits.
Air filter - don't know. Guess I should do that this summer. It's actually not good to clean them very often, they are more effective when they are a little dirty. I think K&N recommends every 15-20k but I can't remember.
If you're not good at remembering this stuff, check out www.mobil1.com - they have an online maintenance journal that you can set up and you tell it to remind you every XX number of miles to replace stuff, and then it emails you and reminds you. Don't forget to rotate your tires

1 - I changed that 4 times in less than 4 years. Not because it needed to be changed but because I've removed something and I needed to drain the coolant.
2 - Every 5000-6000 km
3 - Changed that 3 times in less than 4 years, same reason as #1
4 - I should probably changed those... I still have the sparkplugs that were in the engine when I bought it about 60 000km ago...
