brakes squeeking and grinding

first of all my brakes squeek when stopping slow and then grind when i mash on them. the rotors have 160k on them right now (i think they are original) and they have little cracks in them and i think one has a big crack in it. Ill check it out tonight. the cracks are just through the shiny surface of the rotor and not all the way through. im talking about the front rotors. they warp easily, and make the wheel wobbly, then they unwarp jsut as easily where its smooth again. i checked all the pads and they are a-ok

and suggestions?
