What are the signs? Thanks

Mine went, I had no Idea.
The car is totally drivable.
I can tell you the timing belt had a pretty bad nic in it @ 40K.

Nothing HAPPENED.
I was having my Type S cams put in, and when I picked it up (before I drove it off) I noticed a NIC on the Timing belt, maybe 25% of the way through it. My AEM Cam gears had been pretty eaten up too. It was drivable, and I didn't sense any problems with it. My tuner replaced my cam gears, timing belt and tensioner the next week. If in doubt, ask Honda or someone who knows H22A in and out. It might have to be inspected to be sure. There were no noises that is was making that I could tell. The part is only like $ 150, but labor will kill ya.

The engine will make a rattling sound as the engine revs down at a stand still. Somewhere around 5k to 2k RPMs.

oh ****... serious?? i show these symptoms... to the bat cave!!! i mean honda specialist.

wait help me out... 2 nights ago, i heard metal falling from my car/being run over by my car... whichever... however.. my car started making noise after that... sort of like an exhaust leak.. only closer to my engine.. almost like its coming from my header. i was thinking maybe my gasket cracked, broke, and fell off (hence the metal falling from my car).. but then it doesn't sound like there any exhaust leak from cat back, and it doesn't make a continuous leak. only when my car revs down. i'm thinking you're right 71dsp

I have only heard it when the car is stopped, not in gear, and with no load on it. I don't know what it sounds like with the car in gear (it wasn't my Prelude).

When he took it to the dealer, it was the tensioner. They replaced the timing belt, water pump, and autotensioner, and the noise went away.
